* Summary: Provide patient care and rehabilitation services in a dynamic and supportive environment with opportunities for professional growth and development.

* Assist with patient assessments, develop and implement rehabilitation plans, and interact with patients and families to ensure optimal care outcomes.

* Perform patient care considering needs specific to the standard of care for patient's age, including geriatric and general inpatient rehabilitation patients.

* Under the supervision of the R.N., assist with the assessment on all patients and reassessments as per policy, and identify and initiate appropriate rehabilitation nursing interventions.

* Carry out the plan of care as indicated by the patient's needs and response to treatment, evaluating overall plan daily for effectiveness and updating the interdisciplinary care plan accordingly based on changes in patient's condition and individual needs.

* Interact professionally with patient/family and involve patient/family in the formation of the plan of care, demonstrating ability to adequately assess and reassess pain, and utilize appropriate pain management techniques.


Requirements: graduate of accredited licensed practical/vocational nurse program, current state licensure as Licensed Practical/Vocational Nurse, CPR/BCLS certification, ACLS preferred, IV Certified required or obtained within 6 months, and minimum six months' Medical/Surgical experience in an acute care setting preferred.
